Job summary

Senior Resources of West Michigan is seeking a Registered Nurse Support Coordinator (RN) to join our on-site team in Norton Shores, MI. This full-time exempt role focuses on assessing participants, developing care plans, and coordinating long-term services under the MI Choice program.

Candidates must hold a Michigan RN license, CPR certification, a valid driver's license, and reliable transportation with insurance. The position offers full benefits and pays between $57,222 and $67,626 annually.

Qualifications

Must hold Michigan RN license.
CPR certification required.
Must have a valid Michigan driver's license.
Reliable transportation and current automobile insurance.
TB test on hire and annual symptom review.
Must pass background checks.

Responsibilities

Conduct functional assessment of participant health needs.
Develop an individualized plan of care with participant, wishes, and physician; set frequency/duration of services and implement the plan.
Seek clinical consultation from supervisory staff as needed to assure high-quality plans of care.
Monitor participant's condition and delivery of services; adjust plans as needed; maintain communication with family and physician; advocate for participant.
Maintain participant files and program records.
Review current geriatric and chronic care nursing information to support participant needs.
