Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordination Supervisor in Athens County, OH to lead a team delivering home- and community-based care.

Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology or related field preferred.
• LSW/LPC preferred.
• Three years of experience coordinating supportive services or care coordination.
• Previous supervisory experience strongly preferred.
• Experience in behavioral health and multi-system youth preferred.
• Knowledge of regulations for behavioral health and disabilities.

Responsibilities

• Supervise and oversee activities of care coordination team through meetings, supervision and communications.
• Provide training for direct care coordinators in home and community settings.
• Support staff through individual and group conferences and case problem analysis.
• Promote an ongoing culture of teamwork and recognition.
• Maintain high-fidelity wraparound training credential.
• Ensure supervision of ICC and MCC delivery and documentation.
• Coordinate on-call rotation and participate in fidelity reviews and OhioRISE planning.
• Collaborate with relevant systems to ensure high-quality services with family voice and choice.
• Assess needs in crisis situations and inform response.
• Ensure confidentiality and Medicaid standards.
• Evaluate staff performance and advise manager.
• Assist in staff recruitment and interviews.
• All other duties as assigned.
