Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordination Supervisor (unlicensed) in Ross County, OH. The role focuses on leading a care coordination team and ensuring adherence to high-fidelity wraparound principles, serving children and families from diverse backgrounds.

Ideal candidates hold a Bachelor's or Master’s in a related field with at least three years of supervised care coordination experience and preference for LSW/LPC credentials.

Qualifications

• Bachelor’s degree or higher in social work, education, counseling, psychology, or a related field.
• LSW/LPC preferred.
• Three years of experience providing coordinated supportive services/care coordination.
• Previous supervisory experience strongly preferred.

Responsibilities

• Supervise and oversee activities of care coordination team members.
• Provide training for direct care coordinators in home and community settings.
• Ensure completion of care plans, assessments, and required documentation.
• Maintain adherence to high-fidelity wraparound model and Medicaid standards.
• Collaborate with staff and community partners to ensure quality services.
