Job summary

Wellstar Cobb Hospital is seeking a Senior RN Care Coordinator to assess transitional care needs and coordinate care across the continuum during complex acute admissions. You will engage with patients and families to assure appropriate discharge planning and effective progression of care.

The role includes mentoring new hires, partnering with physicians, PAS, financial counselors, and UM to optimize care and resource use.

Qualifications

• Minimum 3 years of experience in healthcare in the acute care setting or related field.
• Minimum 2 years in care coordination in the acute care setting.
• Knowledge of discharge planning, care progression and post-discharge needs.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage with patient and family to assure care needs are met during complex cases.
• Mentor new hires in clinical progression and discharge planning.
• Collaborate with physicians, utilization review, and care teams to optimize care progression.
