Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN Sr. to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care needs are met for the most complex cases during acute admission.

This role partners with physicians and care teams to drive efficient treatment plans and discharge planning. The RN Sr will mentor new hires, participate in orientation, and support post-discharge planning to promote optimal resource utilization.

Qualifications

• Minimum 3 years of experience in healthcare in the acute care setting
• Minimum 2 years in care coordination in the acute care setting
• Relocation assistance for eligible candidates

Responsibilities

• Assess transitional care needs and coordinate care across the continuum
• Engage with patient and family to assure care needs are met
• Manage discharge planning and post-acute referrals
• Participate in Interdisciplinary Rounds and care conferences
• Mentor/precept new hires and support staff development
