Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN to join the patient services team. The role focuses on coordination of care across acute, chronic, and long-term stages, including utilization management, transitional planning, and patient advocacy.

As a licensed RN, you will collaborate with multi-disciplinary teams, educate patients and families, and ensure adherence to standards of care while supporting service excellence and patient satisfaction.

Qualifications

• Active RN license in Georgia
• Minimum 3 years clinical practice; 3–5 years preferred
• Experience coordinating care across acute, chronic, and long-term care settings

Responsibilities

• Discharge planning with timely resource information
• Identify and document barriers to disposition
• Coordinate care with health-team members
• Provide patient/family education on conditions and care
• Assist with advance directives and post-discharge planning
• Monitor and document care quality indicators
