Job summary

The Wellstar Cobb Hospital Care Coordination team is seeking a Care Coordination RN Sr. to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure complex cases are managed efficiently during acute admission.

The RN Sr will partner with physicians and care teams to drive optimal treatment plans and plan post-discharge care, while mentoring new hires and participating in orientation and precepting as needed.

Qualifications

• Minimum 3 years of healthcare experience in acute care, related field or skilled care.
• Minimum 2 years of care coordination experience in acute care.

Responsibilities

• Coordinate care across the continuum and engage with patient and family.
• Collaborate with physicians and care team to assess progression and discharge plans.
• Identify barriers to discharge and expedite resolution.
• Document assessments and care plans in the medical record.
• Participate in interdisciplinary rounds and care conferences.
