Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN Sr to assess transitional care needs, coordinate across the continuum, and guide complex cases during acute admission. The RN Sr will partner with physicians and care teams to drive efficient care progression and discharge planning.

This role requires an Associates Degree in Nursing, a current RN license with BLS, and minimum 3 years as a staff nurse in acute care plus 2 years in case management; relocation assistance may be available for

Qualifications

• Associates degree in nursing required; bachelor's in nursing preferred.
• Current BLS or ARC/BLS certifications required upon hire.
• Minimum 3 years as staff nurse in acute care; 2 years in case management or hospital-based care coordination.
