Job summary

The Care Coordinator RN Sr. at Wellstar Cobb Hospital is tasked with assessing transitional care needs, coordinating across the continuum, and engaging patients and families to ensure complex cases progress during the acute admission.

You will partner with physicians and care teams to drive efficient care, manage length of stay, and plan post- discharge care. Responsibilities include discharge planning, precepting new hires, and participating in interdisciplinary rounds and care conferences,

Qualifications

• Associates degree or nursing diploma; Bachelors preferred.
• RN license at hire; BLS required.
• Minimum 3 years healthcare experience in acute care.
• Minimum 2 years care coordination in acute care.

Responsibilities

• Assess psychosocial risk factors and discharge readiness with care team.
• Coordinate care progression and discharge planning with physicians.
• Identify and resolve barriers to discharge; expedite care.
• Mentor and precept new hires; support staff in progression.
• Participate in Interdisciplinary Rounds and care conferences.
• Document assessments and discharge plans in the medical record.
