Qualifications

• 3–5 years of experience in a high-touch client-facing role.
• CNA certification is strongly preferred.
• Experience in care management and supporting a team of caregivers.
• Strong communication, interpersonal, and problem-solving skills.
• Knowledge of ADLs and IADLs for senior care clients.
• Experience in private duty/home care service.

Responsibilities

• Conduct in-depth consultations with clients and families to create tailored care plans.
• Guide clients through intake, intake process, and budgeting for in-home services.
• Manage a select caseload and be the main contact for clients.
• Provide emergency coverage for client shifts when needed.
• Support hiring decisions and mentor caregivers to maintain high service standards.
• Match clients with caregivers based on skills and preferences.
• Perform quality assurance visits and follow-up to ensure plan adherence.
• Anticipate evolving client needs and offer proactive solutions.
• Liaise between clients, families, customer service, and scheduling teams.
• Maintain confidential records for clients and caregivers.
• Participate in process improvements and system integrations.
