Open Arms Solutions in the Chicago area is seeking a compassionate Community Care Coordinator to join our private duty home care team. You will work directly with clients and caregivers to ensure individualized plans, high‑quality service, and smooth operations across Oak Brook, Elmhurst and surrounding suburbs.

The role focuses on client consultations, care planning, intake guidance, and caregiver support.

Qualifications

• 3–5 years in a high‑touch, client‑facing role (senior care, private duty or hospitality)
• CNA certification strongly preferred
• Proven care management experience with complex client relationships and leading caregivers
• Strong communication, interpersonal and problem‑solving skills; high discretion
• Knowledge of ADLs and IADLs for seniors
• Experience in private duty/home care service
• Exceptional organizational skills and attention to detail
• Proactive, service‑oriented mindset with a passion for exceptional care
• Proficiency in WellSky or similar care‑management software and MS Office 365
• Ability to lift up to 50 pounds; valid driver’s license and reliable transportation
• Travel up to 50%

Responsibilities

• Conduct in-depth consultations and assessments to tailor care plans.
• Guide clients through intake, care planning and budgeting for in‑home services.
• Manage a caseload as main contact, ensuring high‑quality care.
• Provide emergency coverage when caregivers are unavailable.
• Assist in hiring decisions and mentor caregivers to meet standards.
• Match clients with caregivers based on skills, experience and preferences.
• Perform regular quality assurance visits and follow‑ups.
• Anticipate evolving client needs and offer proactive solutions.
• Act as liaison between clients, families, service teams to resolve issues.
• Maintain confidential records for clients and caregivers.
• Participate in process improvements and system implementations as a super‑user.
