Job summary

Open Arms Solutions is a premier private duty home care provider serving the Chicago area. We are seeking a compassionate Community Care Coordinator to join our team to deliver exceptional care and support in the West Suburbs, at our Lisle Creek Estates facility.

The Care Coordinator reports to the Executive Director and will oversee client care plans, caregiver support, intake, scheduling, and quality assurance. Strong organizational and communication skills are essential.

Qualifications

• 3–5 years in a high-touch, client-facing role, preferably in senior care.
• CNA certification is strongly preferred.
• Proven experience in care management and supporting or leading a team of caregivers.
• Strong communication, interpersonal, and problem-solving skills with discretion.

Responsibilities

• Conduct in-depth consultations and assessments to develop tailored care plans.
• Guide clients through intake, care planning, and budgeting for in-home services.
• Manage a caseload as the main contact, ensuring a high-quality experience.
• Provide coverage for client shifts on an emergency basis.
• Support hiring, training, and mentoring caregivers to meet service standards.
• Match clients with caregivers based on skills and preferences.
• Conduct quality assurance visits and check-ins to ensure plans are followed.
• Maintain confidential records for clients and caregivers.
• Participate in process improvements and system implementations.
