Job summary

PACE by IIH seeks a Center Manager to oversee operations of an IIH center, supervise the IDT, and ensure high-quality participant care. You will coordinate care planning, quality initiatives, and regulatory compliance within a collaborative, community-based healthcare setting.

You will lead staffing, budgeting, and policy adherence while maintaining excellent communication with participants, families, and providers to support the center’s mission.

Qualifications

• Analytical with leadership and decision-making skills.
• Thorough knowledge of health and social services for the elderly, especially home-and-community-based care.
• Knowledge of utilization review, quality assurance, and managed health care concepts.
• Advocate for risk-based long-term care programs (PACE).
• Familiarity with management information systems and fiscal management.

Responsibilities

• Participate as a member of the IDT, facilitate IDT meetings, and provide leadership for care planning and problem solving.
• Develop and implement Quality Improvement activities.
• Ensure center operations meet participant needs and coordinate with IIH departments.
• Supervise IDT staff and center staff; provide guidance to IDT.
• Monitor compliance with grievances, care plans, assessments, and records; implement corrective action.
• Assist in staffing and training of center personnel.
• Oversee service delivery and integration for participants.
• Assist in policy development and adherence across services and budgets.
• Monitor performance trends in utilization, finances and outcomes.
• Participate in IIH management team and regulatory compliance for PACE.
