Job summary

JASA in New York is seeking a compassionate Case Manager for a part-time role two days per week. You will conduct in-home and office-based assessments, develop service plans, and connect older adults to community resources across NYC.

The ideal candidate has a BSW or related degree, experience in eldercare or case management, and the ability to travel NYC via public transit to client homes and agencies. Bilingual Spanish is a strong plus.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Experience in case management or eldercare preferred.
• Willingness and ability to travel NYC via public transit to client homes.

Responsibilities

• Conduct in-home and office-based psychosocial assessments.
• Develop, implement, and maintain individualized service plans.
• Advocate for benefit eligibility; assist with applications and recertifications.
• Coordinate direct services including homecare, transportation, housing, and referrals.
• Evaluate cognitive/physical capacity; escalate to Adult Protective Services when needed.
• Conduct scheduled reassessments; monitor progress and adjust goals.
• Lead community education workshops and represent JASA at events.
• Maintain audit-ready client records in web-based case management systems.
