Job summary

Catholic Charities Brooklyn and Queens, Inc. is seeking a Social Worker II to provide comprehensive case management for frail, homebound older adults, enabling them to remain safely in their own homes. The role involves in-home assessments and care planning for clients with complex needs.

The position requires an MSW and at least one year in human services, plus the ability to travel across the five boroughs and perform crisis intervention when needed. Competitive benefits are offered.

Qualifications

• Master of Social Work degree required.
• One year of experience in human services.
• Knowledge of entitlements for older adults.
• Accredited with Supervision in Field Instruction within 6 months of employment.
• Capable of assisting with more complex client issues.
• Able to travel to multiple locations within the five boroughs as needed.
• Frequently lifts and/or moves up to 10 pounds.

Responsibilities

• Provide a comprehensive in-home assessment and reassessment of the older person’s functional and cognitive capacities, unmet needs, and eligibility for benefits and entitlements.
• Develop and implement a care plan with the senior and/or family to address unmet needs and help them live safely at home.
• Complete a comprehensive DFTA assessment form for each client on a timely basis.
• Maintain up-to-date case notes, waiting lists, and timely submission of monthly statistics and reports.
• Ensure referrals are made and follow up is done to appropriate services.
• Provide crisis intervention services for medical emergencies, utility shut-offs, bedbug infestations, and disaster response.
• Provide emergency coverage for Home Delivered Meal clients when the client fails to answer the door.
