Qualifications

• MSW degree from an accredited program is required.
• LMSW or LCSW licensure in NY preferred; driver's license required.
• 2+ years of experience in social work or geriatric care management.
• Strong knowledge of aging-related services, Medicare, Medicaid and public assistance.
• Proficiency with Microsoft Office and electronic case management systems.
• Excellent interpersonal, communication, advocacy, organizational and time-management skills.

Responsibilities

• Geriatric care management: provide ongoing case management and support.
• Client assessments: in-home visits and develop individualized care plans.
• Resource coordination: access healthcare, housing help, benefits, community resources.
• Client advocacy: collaborate with families, providers and organizations.
• Case documentation: maintain records, monitor care plans and follow-ups.
• Team collaboration: participate in case conferences and quality improvement initiatives.
