Job summary

The Senior Care Options (SCO) nurse case manager at NeighborHealth Corporation acts as a liaison between patients, families and care teams to ensure timely, coordinated care and to minimize fragmentation of services. You will develop patient-centered care plans, assess health status, document in Epic, and collaborate with providers to support independence and safety in the community.

Massachusetts RN licensure and BLS are required; bilingual skills preferred.

Qualifications

• Graduation from an accredited nursing program and RN licensure in Massachusetts.
• BLS certification required; bilingual skills preferred.

Responsibilities

• Assess and coordinate SCO patient care plans with families and providers.
• Maintain accurate documentation in Epic and shared systems.
• Provide patient advocacy and education on care plans.
• Mentor new staff and participate in team meetings.
