Job summary

Department of Elder Affairs is seeking a CARES Assessor to conduct comprehensive assessments for nursing facility placement and home and community-based services. You will evaluate Level of Care and participate in an interdisciplinary team to determine the most appropriate placement.

You will maintain up-to-date medical information, document activities, and input data into CIRTS. Travel within the assigned planning area and reporting to a DOEA office are required.

Qualifications

• Four (4) or more years of professional experience in community and social services or a bachelor’s degree with four years’ experience.
• Ability to evaluate Level of Care and determine appropriate placement.
• Knowledge of state and federal CARES policies and procedures.

Responsibilities

• Coordinate and conduct comprehensive assessments for nursing facility placement and home and community-based services.
• Maintain accurate medical information and document activities regularly.
• Input assessment data into CIRTS and keep records updated.
