Job summary

The Department of Elder Affairs seeks a CARES Assessor to conduct comprehensive assessments for nursing facility placement and home-and-community-based services. You will evaluate clients' Level of Care, coordinate with interdisciplinary teams, and document findings in the tracking system.

In-office role based in the local DOEA office, requiring attention to detail, strong written and oral communication, and the ability to handle sensitive information while maintaining professional demeanor

Qualifications

• Four or more years of professional experience in community and social services, or a related bachelor's degree.
• Knowledge of Level of Care rules, CARES policies, aging process, and community resources.
• Strong written and oral communication with detail orientation and accurate data entry.
• Proficiency in Windows and Office Suite (Word, Excel, Outlook, OneDrive, SharePoint).
• Valid Florida driver's license and ability to operate a vehicle for work travel.
• Ability to work with diverse clients and maintain professionalism.

Responsibilities

• Conduct comprehensive assessments for nursing facility placement and home-and-community-based services.
• Evaluate Level of Care and recommend appropriate placements.
• Coordinate with interdisciplinary teams and maintain accurate client records.
• Ensure compliance with state and federal regulations.
