DEPARTMENT OF ELDER AFFAIRS is seeking a CARES Assessor to perform comprehensive assessments for nursing facility placement and home-based services. The role requires reporting to a local DOEA office and involves evaluating Level of Care for appropriate placement, with a strong emphasis on regulatory compliance and interdisciplinary collaboration.

The position emphasizes data accuracy, timely reporting, and familiarity with CARES policies.

Competenze

• Four or more years of professional experience in community and social services.
• Bachelor’s degree in community and social services or equivalent experience.
• Willingness to travel and possess Florida driver’s license.

Mansioni

• Assess applicants for nursing facility placement and home and community-based services.
• Document medical information and input data into CIRTS.
• Collaborate on Level of Care decisions with interdisciplinary team.
• Ensure PASRR is completed before a Level of Care is issued.
• Participate in disaster recovery and hurricane response.
