Job summary

Department of Elder Affairs in Fort Myers, FL is seeking a CARES Assessor to conduct comprehensive assessments for nursing facility placement and home and community-based services. The role evaluates Level of Care and recommends the most appropriate placement, reporting to a local DOEA office.

The position requires 4+ years in social services or a relevant degree, with travel within the planning area and the ability to work in a high-demand environment while maintaining confidentiality.

Qualifications

• Four (4) or more years of professional experience in the field of community and social services; or
• A bachelor’s degree or higher from an accredited college or university with a major in the field of community and social services; or
• A combination of education and professional experience in the field of community and social services work totaling four (4) or more years.

Responsibilities

• Coordinates and conducts comprehensive assessments of applicants for nursing facility placement and home and community-based services.
• Maintains accurate and up-to-date medical information on clients and inputs data into CIRTS.
• Participates in multidisciplinary teams for Level of Care decisions and PASRR processes.
• Performs duties in accordance with CARES policies, procedures, and regulations.
