Department of Elder Affairs CARES 7A in Florida seeks an experienced Registered Nurse to conduct comprehensive assessments for nursing facility placement and home/community-based services, evaluating Level of Care and advising on the best placement.

The role requires reporting to the DOEA office serving the assigned area and involves occasional travel. Applicants must maintain RN licensure and be prepared for interdisciplinary collaboration and ongoing professional development.

Qualificações

Must hold and maintain a valid RN license.
Willingness to travel and operate own vehicle.
Experience with assessments for care level decisions and interdisciplinary teams.

Responsabilidades

Coordinates comprehensive assessments for nursing facility placement and home/community services.
Maintains medical information and documents activities in CIRTS.
Participates in interdisciplinary teams for Level of Care decisions and PASRR processes.
Ensures PASRR is completed before Level of Care is issued upon state hospital referrals.
Monitors placement decisions and communicates with long-term care network.
Provides medical in-service training to staff on elder care topics.
