The Department of Elder Affairs in Florida is seeking a CARES Assessor to perform comprehensive assessments of applicants for nursing facility placement and home and community-based services. You will evaluate the Level of Care and recommend the most appropriate placement, working as part of an interdisciplinary team.

This non-telework role requires reporting to the DOEA office in the service area; travel is required within the assigned planning and service area.

Qualifications

• Four (4) or more years of professional experience in community and social services.
• Bachelor’s degree or higher in community and social services; or a combination of education and experience totaling four (4)+ years.

Responsabilités

• Coordinates and conducts comprehensive assessments for placement and services.
• Maintains accurate medical information and data in CIRTS.
• Participates in Title XIX Level of Care decisions as part of the team.
• Ensures PASRR process is completed before Level of Care is issued.
• Communicates with the long-term care network for placement and care recommendations.
