Job summary

Qualifications

• Four or more years of professional experience in community and social services, or a related bachelor's degree.
• Knowledge of Level of Care rules, CARES policies, aging process, and community resources.
• Strong written and oral communication with detail orientation and accurate data entry.
• Proficiency in Windows and Office Suite (Word, Excel, Outlook, OneDrive, SharePoint).
• Valid Florida driver's license and ability to operate a vehicle for work travel.
• Ability to work with diverse clients and maintain professionalism.

The role involves coordinating with interdisciplinary teams, maintaining accurate client records in a tracking system, ensuring compliance with state and federal regulations, and participating in emergency response efforts. Key qualifications:

• Four or more years of professional experience in community and social services, or a bachelor's degree in a related field, or a combination of education and experience totaling four years
• Knowledge of Level of Care rules, CARES policies, aging process, and community resources
• Strong written and oral communication skills with detail orientation and ability to enter data accurately into computerized systems
• Proficiency in Microsoft Windows and Office Suite applications (Word, Excel, Outlook, OneDrive, SharePoint)
• Valid Florida driver's license and ability to operate private motor vehicle for work travel
• Ability to work with clients with diverse physical and emotional problems and maintain professional demeanor
