Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to deliver and coordinate primary care for members, primarily in Orange County communities. The role includes in‑home visits, exams, vaccines, and telephonic triage to ensure timely, appropriate care and hospital avoidance.

You will collaborate with case management and PCPs, document thoroughly, and participate in on‑call shifts. Travel is heavy, with 75%+ area coverage, and ongoing professional development is encouraged.

Qualifications

• CA NP license or PA-C in good standing.
• MSN/MPH or equivalent required; geriatric care preferred.
• Wound and Palliative care certification a plus.
• Knowledge of HCC and ICD-10 coding.
• Experience with chronic disease management and care planning.

Responsibilities

• Provide primary care to SCAN members in their residence, often in senior living communities.
• Perform exams, telephonic triage, vaccines, and ordering/interpretation of tests.
• Develop comprehensive care plans with patients, families, and staff.
• Collaborate with case management, PCPs, and specialists for timely care access.
• Document visits and outcomes per protocol; participate in on-call coverage.
• Engage in community events and virtual visits to reduce drive time and improve triage.
• Support quality improvement by addressing care gaps and updating workflows.
