Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for members in their homes or living facilities. You will conduct exams, manage chronic diseases, administer vaccines, and coordinate with the care team to ensure timely, appropriate services.

This patient-facing role requires Master’s level training (APN or PA), AZ licensure, and willingness to travel up to 75% of the time, with a focus on high-quality, compassionate care for older adults.

Qualifications

• Master’s degree in Nurse Practitioner or PA program; education in Adult/Geriatric care preferred.
• AZ NP license or PA-C in good standing; Wound Care and Palliative Care certificates a plus.
• Knowledge of CMS guidelines, ICD-10, and care coordination.
• Ability to manage goals of care, chronic disease management, and patient advocacy.

Responsibilities

• Provide and coordinate primary care for assigned members in residences or facilities.
• Conduct exams, vaccinations, and diagnostic tests; manage urgent visits and triage.
• Develop care plans, address health gaps, and coordinate with case management.
• Collaborate with PCPs and specialists to ensure timely access to services.
• Document visits promptly and participate in on-call coverage.
