Job summary

SCAN is a nonprofit health organization serving older adults across several states. The Advanced Practice Clinician will provide and coordinate primary care to SCAN members, primarily in their residence, to keep seniors healthy and independent.

You will work with a multidisciplinary team to manage chronic diseases, perform exams, administer vaccines, and ensure timely access to services. This patient-facing role requires coordination with care teams, ongoing professional development, and

Qualifications

• Master’s degree in Nurse Practitioner or PA program with geriatrics preferred.
• Wound Care and Palliative Care certificates are a plus.
• Arizona NP license or PA-C in good standing.
• Knowledge of CMS guidelines and Medicare Advantage.

Responsibilities

• Provide care to an assigned panel of members in their residence.
• Conduct exams, vaccines, tests, and triage to prevent hospitalization.
• Develop comprehensive care plans with families and staff.
• Collaborate with case management for timely access to services.
• Document visits promptly per practice protocols.
• Participate in on-call after hours and weekends.
