The role requires at least 2 years SNF admissions experience, strong Medicare/Medicaid knowledge, and proficiency with EMR systems. You will educate families, verify insurance, and collaborate with clinical teams to maintain high-quality resident care.

Qualifications

• Minimum 2 years admissions experience in a Skilled Nursing Facility (SNF).
• Strong Medicare/Medicaid, PDPM, HMO knowledge.
• Familiarity with EMR systems.
• Knowledge of CMS regulations and DOH requirements.
• Excellent communication, organizational, and relationship-building skills.
• Ability to multitask and work effectively in a fast-paced healthcare environment.

Responsibilities

• Coordinate admissions from referral through admission.
• Collaborate with hospitals, physicians, case managers, and referral sources for smooth transitions.
• Develop and maintain relationships with referral partners and community organizations.
• Support marketing/outreach by attending events and community functions.
• Educate prospective residents and families about services, policies, admission procedures.
• Verify insurance coverage, financial information, and authorization requirements prior to admission.
• Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer guidelines.
• Monitor admissions activity and prepare monthly reports on census growth.
• Identify operational barriers affecting admissions and work with leadership on solutions.
• Communicate admission details with interdisciplinary team for continuity of care.
