Job summary

AltaMed Health Services Corporation is seeking an Activities Coordinator in Lynwood, CA, to join the PACE interdisciplinary team. The role focuses on designing, implementing, and overseeing engaging activities that support the physical, cognitive, social, and spiritual well‑being of participants.

The coordinator may use arts, crafts, animals, sports, games, dance, music, and outings. The position requires at least a high school diploma, relevant experience, and completion of the 36‑hour Activity

Qualifications

• Minimum of a High School diploma or equivalent.
• A bachelor’s degree is highly desirable.
• Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population (preferred).
• Experience with dementia care is highly desirable.
• Completion of 36 hours of Activity Leader course, designed specifically for this position and approved by the State of California, is required within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• Experience in providing virtual activities is preferred.
• A valid BLS certification or higher, following AHA or Red Cross guidelines.

Responsibilities

• Design, implement, and oversee activities tailored to the physical, cognitive, and social well‑being of PACE participants.
• Actively participate in care coordination within the IDT and adhere to program guidelines.
• Utilize a variety of techniques including arts and crafts, animals, sports, games, dance, music, and community outings.
• Support the use of remote technology when appropriate for participant comfort and accessibility.
• Maintain a planned schedule of social and purposeful activities and promote a engaging environment.
