Job summary

AltaMed Health Services Corp. is seeking an Activities Coordinator to join the PACE interdisciplinary team. You will design, implement, and oversee activities for participants, including arts, games, movement, and community outings, with remote options when appropriate.

The role requires a high school diploma, 1 year of relevant experience, completion of the 36-hour Activity Leader course within 30 days, and a valid BLS certificate; bilingual candidates are preferred.

Qualifications

• High school diploma or equivalent; bachelor’s degree preferred.
• At least 1 year coordinating services for a frail or elderly population.
• Complete 36-hour Activity Leader course within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.
• Experience providing virtual activities is preferred.
• Valid BLS certification or higher.

Responsibilities

• Participate in care coordination and activity planning for PACE participants.
• Design and oversee engaging activities including arts, games, and outings.
• Utilize remote technology as appropriate to engage participants.
