AltaMed Health Services seeks an Activities Coordinator to join the PACE Interdisciplinary team, coordinating assessments and care planning, and designing engaging programs for participants. The role uses diverse activities—arts, sports, music, and outings—to support physical and social well-being, with possible use of remote tech based on participant comfort.

Reports to PACE Care Center Supervisor. Minimum requirements include a HS diploma, 1+ year of senior/frail population program experience,

Qualifications

• High School diploma or equivalent required; a bachelor’s degree is highly desirable.
• Minimum 1 year of experience in a social or recreational program for frail or elderly population.
• 36 hours Activity Leader course required within 30 days of hire, CA approved.
• Bilingual English/Spanish/Mandarin/Cantonese depending on location preferred.
• Experience with virtual activities preferred.
• Valid BLS certification or higher required per AHA/Red Cross guidelines.

Responsibilities

• Actively participate in care coordination activities, including assessments and care planning.
• Design, implement, and oversee engaging activities tailored to physical, cognitive, social, spiritual and well-being needs of PACE participants.
• Use arts and crafts, animals, sports, games, dance, music and community outings in activities program.
• Maintain scheduled activities and promote an engaging environment for participants.
• Comply with PACE program guidelines, regulatory requirements, and organizational policies.
