WelbeHealth seeks a Behavioral Health Specialist to join its interdisciplinary team supporting the elderly under the PACE model. You will oversee a caseload, conduct assessments and document therapy needs, while collaborating with social work and clinical staff to advocate for participants.

The role requires a Master’s in Behavioral Health or a PhD and CA licensure, with extensive therapy experience, including work with frail seniors.

Qualifikationen

• Master’s Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD) required.
• Licensed by CA Board of Behavioral Sciences as an LCSW or by CA Board of Psychologists as a Psychologist.
• Minimum of five (5) years of behavioral health therapy and assessments experience.
• Minimum of two (2) years of experience with a frail or elderly population.

Aufgaben

• Conduct behavioral health assessments to determine care needs and participate in interdisciplinary team meetings to develop discipline-specific goals and interventions.
• Deliver and document behavioral health interventions as per care plans (symptom management, counseling, crisis intervention, arranging resources, care transitions, individual and group counseling, care management).
• Facilitate individual and group therapies (art/music therapy, psychosocial drama, counseling, discussion) to restore or expand participants’ social roles.
• Provide training on behavioral health symptoms and interventions to WelbeHealth staff.
