Job summary

Pace RI is seeking a Behavioral Health Specialist (LICSW or master’s level licensed clinician) to provide assessment, diagnosis and therapeutic intervention for participants of the PACE program in Rhode Island.

Work is conducted with the Interdisciplinary Team, with responsibilities including counseling, care plan development, documentation in the EHR, and ensuring shared goals of care across teams. Travel to multiple sites as needed.

Qualifications

• 5+ years of direct service work.
• 1–2 years in a medical or geriatric setting.
• 1–3 years of EMR experience (Cognify) preferred.
• RI Licensed Independent Clinical Social Worker.
• MSW degree required.
• Driver's license and travel required.

Responsibilities

• Evaluate participants referred by the IDT via records review and interviews.
• Assess participant, develop a working diagnosis and treatment plan.
• Counsel participants, individually or in groups as part of the treatment plan.
• Guide participants in developing skills and strategies for managing problems.
• Evaluate counseling effectiveness and progress toward objectives.
• Act as a behavioral health resource to the social work team and IDT.
• Participate in organizational behavioral health projects and meetings.
• Provide recommendations for integration of care goals into the care plan.
• Ensure shared goals of care are communicated and documented.
