Job summary

WelbeHealth, LLC. is seeking a Behavioral Health Specialist to work with our interdisciplinary team to oversee a caseload of participants and address behavioral health needs within the PACE model.

The role involves conducting assessments, delivering therapy, documenting care plans, and providing staff training. A master’s degree and relevant licensure are required, plus experience with elderly populations.

Qualifications

• Master’s Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD) is required.
• Licensed by CA Board of Behavioral Sciences as an LCSW or CA Board of Psychologists as a Psychologist is required.
• Minimum of five (5) years of behavioral health therapy and care planning experience.
• Minimum of two (2) years of experience with a frail or elderly population.

Responsibilities

• Conduct behavioral health assessments and participate in interdisciplinary team meetings.
• Deliver and document behavioral health interventions per care plans including therapy and crisis intervention.
• Facilitate individual and group therapies (art/music therapy, psychosocial drama, counseling).
• Provide training on behavioral health symptoms and interventions to WelbeHealth staff.
