State of Arizona in Tucson is hiring a Case Manager 3 – Behavioral Health to support individuals with developmental disabilities and their families within the Division of Developmental Disabilities. The role focuses on coordinating services, assessing needs, and ensuring compliance with ALTCS and state requirements.

The position requires RN, social work, or psychology education, and a valid driver’s license.

Qualifications

• Must have valid driver's license and clean driving record.
• Experience with case management in developmental disabilities or behavioral health preferred.
• Knowledge of ALTCS/DD policies and state regulations.

Responsabilités

• Complete progress notes and ISP documents; maintain organized case files.
• Conduct field visits to family homes, group homes and other placements.
• Assess mental health status, safety, and placement appropriateness; support team goals.
• Attend discharge staffings and hearings; coordinate with mental health agencies.
• Prepare and review cost‑effective studies and administrative reports.
• Lead coordination meetings and supervise coordinators when needed.
