Job summary

The State of Arizona, Department of Economic Security (DES), Division of Developmental Disabilities (DDD) in Benson, is seeking a Case Manager 3 to support individuals with developmental disabilities and their families. The role emphasizes developing PCSPs, coordinating services, and collaborating with community partners to promote independence and community engagement.

Qualified candidates may hold an RN license or a Bachelor's degree in Social Work, Psychology, Special Education, or Counseling

Qualifications

• RN license or Bachelor's in Social Work, Psychology, Special Education or Counseling with relevant case management experience.
• Experience providing case management to elderly or disabled individuals or those with Serious Mental Illness (SMI).

Responsibilities

• Drive a vehicle to conduct onsite visits to members' homes, schools, and program sites to discuss service needs.
• Develop, review, monitor and evaluate Person-Centered Service Plans (PCSP) and document progress and referrals.
• Facilitate meetings and program visits to assist member/family choices; complete referrals for services.
• Collaborate with community agencies to support members and families in crisis.
