The Department of Economic Security (DES) - Division of Developmental Disabilities in Casa Grande, AZ, seeks a Case Manager 3 – Targeted Unit to join our team. This role supports individuals with developmental disabilities and their families through person-centered planning and service coordination.

Qualified candidates will have a relevant degree or RN license, strong knowledge of PCSP, HIPAA, and state policies, plus experience in case management.

Competenze

• Must have a Bachelor's degree in SW/PSY/Special Education/Counseling or an RN license with related case management experience.
• Experience providing case management to elderly or disabled populations and/or individuals with Serious Mental Illness (SMI).
• Valid driver's license and background checks as required.

Mansioni

• Drive to member homes, schools, and program sites to discuss service needs and supports.
• Develop, review, and monitor PCSP with timely progress and referrals.
• Facilitate meetings and visits to assist with member/family choices; refer to services as needed.
• Collaborate with community agencies to provide crisis support and assistance.
• Prepare progress notes, reports, and participate in staff trainings and meetings.
