Job summary

Commonwealth of Pennsylvania's Delaware County Office of Services for the Aging is hiring an Aging Care Manager 3 to assess risk, develop care plans, and coordinate supports for older adults. This full-time Civil Service role in Delaware County, PA offers telework potential after six months, travel for client visits and court hearings, and a comprehensive benefits package.

The starting salary is $52,533 annually; PA residency and a valid driver's license are required, with a Bachelor’s degree

Qualifications

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and a half years of professional human services, law enforcement, or nursing experience with a bachelor’s degree; or
• An equivalent combination of experience and training including six months of relevant experience

Responsibilities

• Risk assessment: gather information to evaluate danger level.
• Report determination: review facts to substantiate the need.
• Care planning: develop plans addressing problems with formal/informal supports.
• Case documentation: maintain complete and timely records in the case system.
• Monitoring visits: conduct home visits and phone contacts to review care plan effectiveness.
• Emergency response: arrange urgent shelter and support court actions when risk cannot be reduced.
