Qualifications

• An equivalent combination of experience and training with at least six months in related fields.

Responsibilities

• Case Investigation: Review and investigate referrals to determine client risk and eligibility for services.
• Information Gathering: Obtain psych/medical assessments and financial details.
• Client Assistance: Provide guidance to older adults and families to resolve problems and improve circumstances.
• Professional Collaboration: Work with staff, agencies, and community partners through home visits and meetings.
• Documentation: Maintain accurate records and entries in the standard system.
• Court Intervention Support: Prepare docs for cases requiring guardianship or protective actions.
