Job summary

The Delaware County Office of Services for the Aging is seeking an Aging Care Manager 3 to assess risk, develop care plans, and coordinate supports for vulnerable older adults. This full-time role requires careful documentation, timely case notes, home visits, court coordination, and occasional travel; telework may be available after six months.

Qualifications

Minimum one year as Aging Care Manager 2 or Aging Program Assessor or equivalent experience.
One and one-half years of professional human services, law enforcement, or nursing experience with a related degree.
Equivalent combination of education and experience accepted.

Responsibilities

Risk assessment using information from multiple sources to gauge danger levels.
Review facts to determine substantiation of the report of need.
Develop care plans addressing presenting and underlying problems with formal/informal supports.
Maintain complete, timely records of investigative activities and contacts.
Conduct home visits and phone contacts to monitor safety and plan effectiveness.
Arrange urgent shelter and support court actions when risks cannot be reduced.
