County of Northampton seeks an Aging Care Manager 3 – Protective Services to maintain a caseload of vulnerable older adults needing protection or mental health case management services.

The role reports to the Aging Care Management Supervisor and requires documentation, investigation, coordination of services, and adherence to confidentiality in accordance with Local Plan for Protective Services and state regulations.

Qualifications

• Knowledge of casework principles and methods.
• Knowledge of resources for older adults.
• Ability to work with older adults with various problems.
• Ability to communicate clearly in English and possibly Spanish.

Responsabilités

• Receives and reviews Reports of Need regarding older persons referred for protective services.
• Initiates investigation in accordance with time frames and state regulations.
• Completes standardized electronic forms for investigation, assessment, and service plan.
• Advices consumer about protective services and obtains informed consent.
• Makes referrals and coordinates delivery of needed assistance.
• Maintains confidentiality and participates in court or hearings as needed.
