Qualifications

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• 1.5 years of professional human services, law enforcement, or nursing; or
• equivalent combination including six months in human services/law enforcement/nursing.
• Resident of Pennsylvania.

Responsibilities

• Record Reports of Need and determine category and priority.
• Investigate all abuse allegations within regulatory time frames.
• Conduct Face-to-Face visits with older adults within regulatory time frames.
• Interview collateral contacts and collect evidence and documents.
• Work with external agencies to reach case conclusions.
• Determine substantiated or unsubstantiated outcomes.
• Arrange for Aging services, medical intervention, protective orders, and placement as needed.
• Assist with entitlement programs (LIHEAP, Medicaid, SNAP, etc.).
• Testify in court if needed and conduct reassessments and final closings.
• Document activity in the Well-Sky database and participate in after-hours coverage.
