Job summary

Community Horizons is seeking a Support Broker to empower individuals with disabilities and older adults to live independently. You'll recruit and mentor caregivers, coordinate care teams, and guide clients through complex healthcare and long-term care systems.

Qualifications

• Healthcare recruiting experience is required.
• 5+ years supporting individuals with disabilities or frail elderly.
• Valid driver's license and reliable transportation.

Responsibilities

• Attend referral meetings and conduct in-home visits with members and families.
• Help individuals recruit, screen, and hire their own caregivers or nurses.
• Support members through interviews, onboarding, and retaining quality staff.
• Mentor members on how to be successful employers, including managing schedules and staff expectations.
• Guide individuals through scheduling, timecards, and Electronic Visit Verification (EVV) systems.
• Assist members with Medicaid renewals, benefit applications, and navigating public assistance programs.
• Help individuals understand and navigate complex healthcare and long-term care systems.
• Maintain documentation and case notes while providing ongoing mentoring and support.
• Advocate for the individual's voice, preferences, and independence in every step of the process.
