Job summary

Community Horizons in Shawano, WI is seeking a Support Broker to empower individuals to build their own care teams, enabling independent living. You’ll conduct in-home visits, coach members on hiring caregivers, and guide scheduling, timecards, and EVV systems.

This role emphasizes person-centered advocacy and collaboration with families, with a semi-remote arrangement and mileage reimbursement. Join a mission-driven team helping people live with dignity and choice.

Qualifications

• Healthcare recruiting experience (required).
• 5+ years supporting individuals with disabilities or the frail elderly.
• Strong organization and communication skills.
• Passion for person-centered support and advocacy.
• Experience with self-directed or long-term care programs (a plus).
• A valid driver’s license and reliable transportation.

Responsibilities

• Attend referral meetings and conduct in-home visits with members and families.
• Help individuals recruit, screen, and hire their own caregivers or nurses.
• Support members through interviews, onboarding, and retaining quality staff.
• Mentor members on how to be successful employers, including managing schedules, communication, and staff expectations.
• Guide individuals through scheduling, timecards, and EVV systems.
• Assist members with Medicaid renewals, benefit applications, and navigating public assistance programs.
• Help individuals understand and navigate complex healthcare and long-term care systems.
• Maintain documentation and case notes while providing ongoing mentoring and support.

Your Role: Support Broker

As a Support Broker, you are more than a recruiter—you are a partner, mentor, and advocate helping individuals build the team that makes independent living possible.

You’ll work alongside members and families to ensure they have the tools, support, and confidence to lead their own care and navigate complex systems that impact their independence.

What You’ll Do

You’re a Great Fit If You Have
