Job summary

Community Horizons is seeking a Support Broker in Wisconsin to empower individuals with disabilities and older adults to live independently. You will help people recruit and manage their own care teams, guide them through complex care systems, and advocate for their choices and dignity.

Responsibilities include staffing, onboarding, EVV navigation, Medicaid renewals, and maintaining detailed case notes. A strong preference is given to candidates with self-direction program experience and a valid

Qualifications

• Healthcare recruiting experience (required).
• 5+ years supporting individuals with disabilities or the frail elderly.
• Strong organization and communication skills.
• A passion for person-centered support and advocacy.
• Experience with self-directed or long-term care programs (a plus).
• A valid driver’s license and reliable transportation.

Responsibilities

• Attend referral meetings and conduct in-home visits with members and families.
• Help individuals recruit, screen, and hire their own caregivers or nurses.
• Support members through interviews, onboarding, and retaining quality staff.
• Mentor members on how to be successful employers, including managing schedules, communication, and staff expectations.
• Guide individuals through scheduling, timecards, and Electronic Visit Verification (EVV) systems.
• Assist members with Medicaid renewals, benefit applications, and navigating public assistance programs.
• Help individuals understand and navigate complex healthcare and long-term care systems.
• Maintain documentation and case notes while providing ongoing mentoring and support.
• Advocate for the individual's voice, preferences, and independence in every step of the process.
