Job summary

Senior-Helpers- is seeking a compassionate Client Advocate to join our non-skilled private duty home care team in Wisconsin. You will be the primary contact for prospective and current clients, families, facilities, caregivers, and staff, handling leads, assessments, safety checks, and follow-up to ensure timely updates and strong relationships.

The ideal candidate is organized, detail-oriented, and able to work independently while managing multiple clients.

Qualifications

• Experience in home care, senior care, healthcare, or client relations.
• Strong written and verbal communication.
• Excellent organization and follow-up abilities.
• Ability to travel locally and conduct home visits.

Responsibilities

• Respond promptly to new client leads and inquiries.
• Build rapport with prospective clients, families, and referral sources.
• Gather client needs, schedules, and service preferences.
• Follow up consistently throughout intake.
• Convert inquiries into new clients with a positive first contact experience.
• Conduct initial client assessments to understand needs and preferences.
• Identify appropriate non-skilled services based on client needs.
• Document assessment information clearly for the team.
• Coordinate with families and referral sources to set expectations.
• Visit homes or facilities to assess safety and accessibility.
• Perform environmental checks and identify safety concerns.
• Communicate findings and help coordinate solutions.
• Ensure environments are safe for non-skilled care.
• Serve as a consistent point of contact for clients and families.
• Keep families updated on services and changes.
• Respond to questions in a timely, compassionate way.
• Help resolve concerns and escalate when needed.
• Build lasting relationships with clients, families, and referral partners.
• Maintain positive relationships with facilities and referral partners.
• Provide timely updates regarding clients and services.
• Coordinate communication among facilities, families, caregivers, and leadership.
• Support referral relationship development.
• Provide regular updates to Client Relations Coordinator.
• Share important client information with care and admin teams.
• Monitor follow-ups and ensure tasks are completed.
• Identify potential client satisfaction issues early.
