Job summary

Livingston County Community Mental Health Authority is seeking a Self-Determination Coordinator I/II to support individuals and families in exercising choice and control over services. This role requires collaboration across staff, EORs, FMS agencies, guardians, and providers.

You will educate consumers on options, manage budgets and agreements, and ensure Medicaid compliance. Strong communication and data skills are essential for success in a dynamic public health setting.

Qualifications

• Must have strong written and verbal communication.
• Experience coordinating multidisciplinary teams and stakeholders.
• Ability to analyze data, prepare reports, and support budget development.
• Knowledge of MDHHS policies and self-determination concepts preferred.

Responsibilities

• Coordinate the full network supporting self-determined arrangements across multiple partners.
• Issue, track, and monitor self-determination agreements and budgets.
• Develop policies, standards, and quality improvement indicators for the program.
• Lead training and orientation for staff, families, and individuals.
