Job summary

North Bay Regional Center is seeking a Participant Choice Coordinator to expand the Self-Determination Program and SDP referrals. You will train staff, assist families, and streamline budgets and spending plans to ensure compliant implementation.

The role emphasizes collaboration with case management and fiscal teams, with emphasis on timely service access and performance in a diverse community in California.

Qualifications

• Bachelor's degree and at least two years’ experience working with individuals who have developmental disabilities.

Responsibilities

• Provide referrals to Self-Determination Orientation
• Work closely with Case Management to develop the Self-Determination Budget
• Review Spending Plan to assure alignment with applicable laws, regulations and directives
• Organize and maintain tracking system SDP referrals and enrollments
• Entering information into database
• Coordinate meetings for new participants and renewals to meet timelines
• Participate in outreach events
• Ongoing communication with Case Management, Fiscal, SDP participants, families, Independent Facilitators (IF) and Financial Management Services (FMS)
