Job summary

Livingston County Community Mental Health Authority is seeking a Self-Determination Coordinator I/II to support individuals and families in exercising choice and control over services. This role requires collaboration across staff, EORs, FMS agencies, guardians, and providers.

Starting Compensation:

Self Determination Coordinator I $66,094.50 - $75,996.99

Self Determination Coordinator II $71,778.63 - $82,532.74

Salary based on education and experience.

Role:

Livingston County Community Mental Health Authority (LCCMHA) is seeking a Self-Determination Coordinator I/II to join our Individuals with Intellectual and Developmental Disabilities Program. This role sits at the center of how individuals and families exercise choice and control over their own services, and it requires someone who can move fluidly between direct education and support, program administration, and quality oversight.

What You'll Do:

You’ll coordinate across the full network that supports self-determined arrangements, including LCCMHA staff, Employers of Record (EOR), Managing Employers of Record, Financial Management Service (FMS) agencies, Purchase of Service providers, guardians, support people, and internal committees. Day to day, that means educating consumers and families on self-determination principles and their options, including individual self-determination, agency self-determination, agency with choice, and CMH contracted provider models, so people can make informed decisions about their care.

On the administrative side, you’ll issue and track self-determination agreements, formulate and monitor budgets, and analyze data and trends to keep the program accountable and effective. You’ll ensure compliance with Medicaid requirements and MDHHS policy, track provider qualification standards, and serve as the agency’s point person for state, federal, PIHP, and MDHHS audits, including document collection and tracking.

You’ll also lead training and orientation for LCCMHA staff, EORs, families, and individuals, and facilitate informational meetings onsite, in the community, or in a person’s home. Beyond direct coordination, you’ll develop policies, standards, and quality improvement indicators, manage quality assurance processes for the self-determination program, and bring performance data and improvement recommendations directly to leadership.

This role calls for working knowledge of Person-Centered Planning, Independent Facilitation, the Habilitation Supports Waiver, Supports Broker services, Goods and Services, and Medicaid supports, along with familiarity with the DSM-5 and its clinical application. You’ll engage in interdisciplinary team consults and represent the agency at regional and state meetings, so strong written communication and the ability to synthesize complex information into clear, actionable material are essential.

Qualifications and Experience:

Education: Possession of Master’s Degree in Social Work, Psychology or related field.

Licensure:

• If degree is in Social Work, prefer a Michigan Licensed Master’s Social Worker (LMSW). Will consider a Limited Licensed Master’s Social Worker (LLMSW) with experience.
• If degree is in Counseling, prefer a Michigan Licensed Professional Counselor (LPC). Will consider a Limited Licensed Professional Counselor (LLPC).
• If degree is in Psychology, prefer a Michigan Limited Licensed Psychologist (LLP) or Michigan Licensed Psychologist (LP).

Experience:

• Three (3) years of administrative experience within a Community Mental Health agency or similar publicly funded agency, preferably with self-determination.
• Current knowledge of MDHHS self-determination policies and guidelines.
• Demonstrated experience in the development and monitoring of individual budgets.
• Experience in program development and evaluation, project and data collection and management, strategic planning and group facilitation desired.
• Current knowledge of MDHHS, PIHP, CMHSP regulations and policies.
• Current knowledge and understanding of MDHHS Behavioral Health Code Chart and Provider

Qualifications:

• Experience including working with and managing external partners/providers is preferable.
• Experience with contract management functions/agreements, preferably self-determination agreements.
• Skill in research, data analysis, and report preparation.
• Ability to organize work, set priorities, manage multiple projects, and meet deadlines.
• Demonstrated proficiency in Microsoft Office and electronic medical records systems.
• Ability to communicate effectively in person, by phone, and through telehealth technologies.
• Excellent customer service skills.
