Job summary

Montgomery County Coalition for the Homeless is seeking a Case Coordinator for a seasonal role in Rockville, MD, focused on overflow clients. You will use housing-first and trauma-informed care, applying motivational interviewing to move clients toward permanent housing.

You will manage caseloads, assess needs, provide referrals to health, vocational, education, and substance-use programs, document in HMIS, and coordinate with clinical and operations teams.

Qualifications

• A bachelor's degree in social/human services or family services is required; a high school diploma with related qualifications may be considered.
• One year of hands-on experience in social/human services with diverse populations.
• Excellent written and spoken English communication.
• Solid outreach, vocational counseling and/or job training experience.
• Strong writing, referral and community collaboration skills.
• Good organization in documentation, service planning and follow-up.
• Bilingual in one or more languages preferred.
• Proficient with Microsoft Office and online benefits applications.
• Knowledge of Housing First, Harm-Reduction, and Trauma-Informed Care.
• Knowledge of CPR and minor first aid.
• Valid driver’s license and clean driving record.

Responsibilities

• Maintain an active caseload of overflow clients who need housing-focused case management.
• Assess overflow clients’ needs and implement service plans.
• Assist clients with moving to permanent housing within 30 days when possible.
• Provide referrals to health, psychiatric, vocational, educational, and substance use programs and housing options.
• Request transportation for clients to appointments and update HMIS documentation.
• Complete Acuity Scale and VI-SPDAT and maintain client records for HMIS reporting.
• Meet with clients weekly and document Case Notes in HMIS.
• Collaborate with Clinical Director, Lead Case Manager, and Operations Team.
